Intensive Care Prioritization Protocol: “It really is a disaster plan”

Prioritization protocol for intensive care: & ldquo; This is truly a disaster plan & rdquo;

MISE & Agrave; DAY

With hospitals overflowing more and more, Quebec could be forced to declare an emergency protocol in hospitals if the situation continues to worsen. & nbsp;

“The prioritization protocol for intensive care is a protocol of last resort. You have to have reached a level of 200% of its capacity, so we are not there at the moment (…) it is really a disaster plan ”, says Marie-Ève Bouthillier, manager at the center. of ethics of the CISSS de Laval.

Normally, the system tries to help the patients who need it most, but if we got to the stage of initiating the emergency protocol, we would adopt a different mindset.

“We have reached version 3 of this protocol. You have to have it all over the world. It's the same principle everywhere. We are trying to identify the individuals who are most likely to be successful if they go to intensive care, “explains Ms. Bouthillier.

The decision to initiate this protocol must come from the ministry, hospitals cannot make this decision individually. This prioritization system for intensive care would ensure that a very strict plan is established and the fact of being vaccinated or not would not influence the level of care.

“If you don't have a lot of resources, we will give them to people who will benefit from them. So people who would have a very high death rate, say more than an 80% chance of dying in intensive care, might be un-prioritized as a first step, ”she says. & Nbsp;

The first wave in Italy

This very precise plan ensures that doctors are not left with the pressure of choosing between two patients. It is the protocol that will decide according to certain criteria. Italy had to call for an emergency protocol during the first wave since the pressure was becoming too intense on health personnel.

“Italy probably set the tone for the states to say prepare yourself because this could happen to you this overflow and if it overflows it must be done in a structured way, ”explains the person in charge of the ethics center of the CISSS de Laval.
